Credential Documents Upload Form

This form is to be used by credential candidates enrolled in the teacher preparation programs at the Kalmanovitz School of Education (KSOE). Candidates should use this form when they have completed all requirements of the program and are ready to submit for their credential. Documents that are not official will be rejected and will not fulfill the requirement. Additional information regarding the credential process as well as documents required per your credential program, can be found through the Credential Office website.

Requirement

Supporting Documents

Qualifying CBEST, CSET Multiple Subjects I, II, III + Writing Skills, SAT, ACT, AP scores, or coursework may fulfill the requirement. If you would like to fulfill BSR through coursework, please email credential@stmarys-ca.edu to request for Basic Skills Evaluation.

Qualifying CSET scores or degree majors may fulfill the requirement. Students who earned a degree in JCL-MSTE and JCL-SPED will fulfill the Subject Matter requirement for MSTE and SPED credentials and may upload their unofficial transcript to fulfill this requirement. Please note that the Credentials Office will verify official transcripts and degree conferrals internally. If you are unsure about whether your degree major qualifies for fulfilling SMCR, please email the Credentials Office at credential@stmarys-ca.edu.

A PDF or screenshot of your CTC credential record showing valid documents will fulfill the requirement.

Coursework that covered the principles of the Constitution will fulfill the requirement, under the condition that the course was taken for 2 semester or 3 quarter units and received a grade of B- or better. Unofficial transcripts may be submitted by the student, but SMC must be able to verify this coursework through official transcripts.

A PDF or screenshot of your unofficial SMC transcript with complete credential coursework and grades posted on GaelXpress will fulfill the requirement. An unofficial transcript from the Registrar's Office will also fulfill the requirement.
