Application Forms

Submit your application as a single file with your last name_application as the name of the file (e.g. Smith_application.pdf). Submit any appendices to your applications as a single PDF or Word file with your last name_appendices as the name of the file (e.g. Smith_appendices). Submit a single pdf including CITI training certifications for all researchers included in the project. If you need to scan printed material to convert it to PDF, there is a scanner available in the SMC library. If you are a student, your supervising faculty should submit your application.

The spring of 2024 was the final semester in which we accepted IRB applications through the Moodle platform. We are using Google Forms the semester.

Remember to submit all three pdf's - one for the application, one for the appendices, and one for the CITI training certificates.
